Application for Radioactive Materials License

Use the application form to apply for a new license or amend/renew a current license. Attach additional pages as necessary. Please see the Licensing Guides Tab for the detailed instructions for application specific details for your specific modality.

If you are subject to Part 37, please fill out the Part 37 Commitment Form .

The preferred method of submission of non-security related application materials is via email to [email protected] with ‘Application – GA xxxx-x’ in subject line (if your License number is known). If you are a new licensee or are replacing an RSO or Authorized user, please also send a signed paper copy to the address listed on the application.

An Application Fee (indicating your facility name and address) should be sent to the mailing address on the application form. The Fee Table is available here.  A fee is NOT necessary if you are submitting a Renewal Applicant whose license has not expired, you do not need to submit an application fee.
