Additional SIP Permitting Information

  • Name Change Form
Submit only this form if the facility is changing its name.
  • Change in Ownership Form
Submit only this form if the facility is changing owners. If a facility name change is also occurring with the change in ownership, do not submit the Name Change Form. Note: Only the first three sections (through the Applicable Rules section) of the Title V application should be completed through GEOS.
  • SIP Exemption Attachment
Include this attachment with the SIP Application, if the facility has performed any past exempt permitting activities according to Georgia Air Quality Rule 391-3-1-.03(6)(i)3.
