Solid Waste Permit Application Supplemental Form
The following form must be filled out and submitted alongside your application submittal in GEOS. For questions with GEOS Submittals, contact the Solid Waste Technical Assistance Duty Officer.
Contact the Solid Waste Technical Assistance Duty Officer for Permit-by Rule (PBR) notification forms for Transfer Stations, Collection, Yard Trimmings Landfills, On-site Processing, and Thermal Treatment facilities. PBR notifications may be submitted in GEOS after contacting the technical duty officer or by submittal to the address listed on the form.
NOTE: Permit-by Rule for Class 2 Composting Facilities must now be submitted through GEOS. These notifications are reviewed by by the Recovered Materials Unit. For questions with PBR submittals for Class 2 Composting, please email [email protected].
