1.00 General Information
Must be completed and submitted with ALL Georgia SIP Air Permit Applications, along with any other relevant forms listed below.
Emission Unit Forms
- Form 2.00 Emission Unit List
Must be included if requesting the construction, operation, and/or modification of any emission unit. - Form 2.01 Boilers and Other Fuel Burning Equipment
Must be included if requesting the construction, operation, and/or modification of any fuel burning equipment (boiler, burner, engine, turbine, dryer, smelter, etc.) excluding incinerators. - Form 2.02 Organic Compound Storage Tanks
Must be included if requesting the construction, operation, and/or modification of any organic compound storage tank greater than 4000 gallons. - Form 2.03 Printing Operations
Must be included if requesting the construction, operation, and/or modification of a printing press. - Form 2.04 Surface Coating operations
Must be included if requesting the construction, operation, and/or modification of any surface coating process, excluding Printing Operations. - Form 2.05 Waste Incinerators
Must be included if requesting the construction, operation, and/or modification of a solid/liquid waste destruction operation. Must be included if requesting the construction, operation, and/or modification of the manufacturing process.

Must be included if requesting the construction, operation, and/or modification of an Air Pollution Control Device. - Form 3.01 Scrubbers

This form is to be completed and submitted with any Georgia SIP Air Permit application for the construction, operation, and/or modification of process equipment, fuel burning equipment, and/or air pollution control equipment. - Form 5.00 Monitoring Information
Must be included if requesting the construction, operation, and/or modification of an emission unit(s)/source(s) that discharges air pollutants into the atmosphere through a stack(s) and has monitoring equipment installed or monitoring required (Visible emission checks). -

Must be included if the facility has any fugitive emission sources -

7.00 Air Modeling Information
Must be included if the request involves the construction, operation, and/or modification of an emission unit(s)/source(s) that discharges air pollutants into the atmosphere generally through a stack(s) and requires computer air modeling.

Change Form
Submit only this form if the facility is changing its name. -
Change
in Ownership Form
Submit only this form if the facility is changing owners. If a facility name change is also occurring with the change in ownership, do not submit the Name Change Form. The Change in Ownership Form handles both. If the facility is a Title V source, then an updated Title V application will need to be submitted IN ADDITION to the appropriate SIP Name/Ownership Change form. Note: Only the first three sections (through the Applicable Rules section) of the Title V application should be completed through GEOS. - SIP Exemption Attachment
Include this attachment with the SIP Application, if the facility has performed any past exempt permitting activities according to Georgia Air Quality Rule 391-3-1-.03(6)(i)3. - Download Entire Package (self-executable Zip™ file)
